This form is for use only by defence counsel after the Court has ordered an Indigenous Background Report (previously referred to as a Gladue report) be prepared for sentencing purposes.

Note: All successfully submitted Indigenous Background Report Request forms will generate an automated CONFIRMATION email, sent to defence counsel's email address as entered on this form. If you do not receive a confirmation email, your form may not have been received.
